Submitting parking lots to the data base

In order to submit a parking lot to the data base, a form should be downloaded from this site, filled up and sent to the administrator by fax or e-mail (address is given in the 'Contact' section).

Information about filling in the form

Mileage (expressed in kilometres) defining placing of the parking lot on the road (or place of entering of the road leading from a parking lot to the national road) should be given exact to 100 m, for example: km 126,7
In case of parking places out of cities, on indicatory sketch, it is enough to give two the nearest localities, between which the parking lot (or the entrance to it) is located, and the symbol of a parking lot, allowing to explicitly define the side of the road, by which the parking place is located (do not draw the symbol of the parking lot on the road)
Please, add at the end of the form information about kinds of bank cards and credit cards accepted in the parking, especially DKV, ESSO, UTA cards used by transport companies.

For safety reasons, the document has its extension changed. In order for MS WORD editor (or other editors accepting this format) to accept it automatically, one should after downloading it change the extension of the file to *.doc (for example: Informator-Ankieta-1.doc)
